Week of October 12, 2026 · Healthcare Weekly Brief
Star Ratings crowned a new Medicare Advantage winner, UnitedHealth raised guidance for the third time and investors fled into healthcare as inflation hit 3.7%, while hospitals and ACA enrollees face a costlier 2027.
01
The Setup
2027 Star Ratings made Humana the top Medicare Advantage beneficiary as UnitedHealth beat Q3 and raised 2026 guidance to at least $20.50. A hot September CPI (3.7%) pushed the 10-year to about 5.28% yet health care led the S&P 500 on Oct 14 as investors rotated out of tech. Hospitals sued again over the 340B rebate pilot, Stryker bought into CMR Surgical, and 2027 ACA premiums are set to rise about 15% at the median.
01
95% of Humana's MA members land in 4+ star plans for 2027, up from 20%. HUM jumped 11.6% Oct 9; UNH and CVS likely lose 4+ star share.
02
Q3 adj EPS $4.69, guidance at least $20.50. But a $610M reserve release helped, and commercial trend is still above 11%.
03
CPI hit 3.7% and the 10-year rose to ~5.28%, yet health care led the S&P 500 up 2.44% as money left tech. Biotech stayed flat.

The Intelligence Take
Analyst assessment
Treat Humana's Stars win and UnitedHealth's raise as real but priced, and don't confuse a rate-driven rotation into big pharma and payers with a fundamental all-clear; cost trend, the Fed and 340B still decide 2027.
01Stars, not earnings, reset MA economics this week. Humana's jump from 20% to 95% of members in 4+ star plans is worth billions in 2028 quality bonuses, and it arrives as AEP opens Oct 15 with average MA premiums falling to $12. The open question is how much of that windfall Humana spends on richer benefits to win share back from UnitedHealth and CVS, both of which are likely to see their 4+ star share fall.
02UnitedHealth's beat was real but flattered. Strip out the $610M reserve release and the medical care ratio was roughly 89.1%, still better than a year ago, but commercial trend above 11% and a guided 'low end' for 2027 growth tell you the cost cycle is not over. Employers and ACA buyers are the ones absorbing it: 15% median proposed exchange hikes follow 20% in 2026, Cigna is out of the exchanges for 2027 and enrollment is drifting toward an older, sicker pool.
03The market's message on Oct 14 was rotation, not fundamentals. Health care rallied on a hot CPI day while the 10-year rose, which is the opposite of how rate-sensitive pharma usually trades. That bid favors large-cap, cash-generating names (UNH, LLY, MRK, PFE) and leaves small biotech behind: XBI was flat and is down 2.3% for the week. We got this wrong last week, too. Our bear case expected a hot CPI to hit payers and biotech together, and payers rallied instead.

Regulatory Landscape
FDA
October's PDUFA calendar is light; Daiichi Sankyo and Merck withdrew the I-DXd BLA for small cell lung cancer (Oct 10 date no longer pending). Next: GSK/Ionis bepirovirsen Oct 26, Summit ivonescimab Nov 14, Capricor deramiocel Nov 22. Vykat XR (Neurocrine) faces renewed safety scrutiny with 7 deaths and 100+ serious events in FAERS, causality undetermined.
CMS
2027 Star Ratings released: about 71% of MA drug-plan enrollees in 4+ star plans, average 3.99. Medicare open enrollment runs Oct 15 to Dec 7 with average MA premiums projected at $12.00 (from $14.37) and 34M MA enrollees. HRSA's revamped 340B rebate pilot starts in January; HRSA estimates $537M in annual costs, which hospitals say exceeds $1B.
LEG
No major federal healthcare bill moved this week. A bipartisan bill to recruit rural health workers was introduced; an Axios-Ipsos poll found Medicare for All supporters outnumber opponents nearly 2 to 1 ahead of the midterms. Enhanced ACA tax credits remain expired, driving 2027 premium increases.
